Assessment Recording and Tracking

The type of assessment tracking process put into place by a centre will be determined by a number of factors, including the type and volume of curriculum offer.

Centres will need to develop their own documentation to support this process, examplar forms have been included that can be used or updated as required.

Planning

Example - Assessment planning

Used when implementing the assessment strategy, to ensure the assessment methods being used are appropriate for the target group of learners, the level and the credit value of the unit.  The form is completed by the assessor and can be made available to the internal verifier as part of the internal centre standardisation process.

Recording evidence

Example - Individual Witness Statement

Example - Group Witness Statement

These example forms can be used to document evidence which arises when undertaking assessment activities.

A witness statement can be used to document naturally occurring evidence which meets one or more assessment criteria.  Two examples of the same form have been included above depending on the number of learners undertaking the exercise.

Example - Assessor's Digital Recording Record

Used as a log of digital recording of learners work either individually or within a group across a number of assessment activities.

 

Tracking Learner Progress

The two example forms can be used at two different levels to track a learner’s progress in attaining a unit.  Secondly to track a cohort of learners as they accumulate units and document their achievements against a course or qualification.

Example - Individual Learner Record for Unit Achievement

Used to track an individual learner's progress in completing an individual unit.

Example - Assessor's Record of Learner's Achievement

Used to track a group of learners' progress in completing an individual unit.

 

Learner Feedback

Example - Assessor Feedback to Learner

Ongoing feedback to learners not only provides evidence of a learner’s progress towards meeting the requirements of a unit but also provides a process for the assessor to identify areas that require further work and give the learner an opportunity to understand the requirements in achieving the unit.

The form can be incorporated into the learner’s portfolio to record feedback process to a learner and provides a good source of evidence for the internal and external verification process.
